The NHS Mental Health Crisis: Why So Many Are Going Private
1.7 Million on Waiting Lists
NHS mental health waiting lists stand at approximately 1.7 million people in England. Community mental health waits can stretch close to two years for those waiting the longest — more than twice the wait for equivalent physical health treatments.
Waiting Makes Things Worse
80% of patients said their mental health worsened while on an NHS waiting list. What begins as manageable anxiety can escalate to a debilitating panic disorder. Untreated depression deepens. Waiting is not neutral — for mental health conditions, delay has a direct clinical cost.
Specialist Care Takes Longest
For conditions requiring a consultant psychiatrist — bipolar disorder, severe depression, personality disorders, complex PTSD — NHS specialist waits are among the most extreme. Many patients are referred and wait 6 to 24 months before seeing a specialist who can diagnose and begin treatment.
Private: Seen Within Days
With a FastDoc referral letter, a private consultant psychiatrist can typically see you within days to a few weeks. An initial consultation produces a diagnosis, treatment plan, and where appropriate, a comprehensive treatment plan — rather than a letter telling you how long to wait.

Do You Need a Referral Letter for a Private Psychiatrist?
You need a referral letter if...
You have AXA Health insurance — AXA requires a GP referral before authorising consultant psychiatrist assessments and issuing a pre-authorisation code
You have Aviva insurance — Aviva follows the standard GP referral pathway for specialist mental health care
You are booking at a private hospital such as Spire, Nuffield, or HCA — hospital psychiatric departments typically require a clinical referral letter
You want the psychiatrist to have full clinical context before your appointment — a referral letter means more productive first session and faster diagnosis
You have a complex or long-standing condition where documented clinical history significantly improves the quality of your first consultation
You may not need one if...
You have Bupa insurance — Bupa offers a Direct Access pathway for mental health that does not always require a GP referral letter, though some policies and conditions do require one
You are self-paying and booking directly with an independent private psychiatrist — many accept self-referrals from self-pay patients
Your chosen psychiatrist or clinic explicitly accepts self-referrals and you are not using insurance to cover the cost
Always check your specific policy and obtain pre-authorisation from your insurer before booking — booking without authorisation is the most common reason psychiatric claims are declined.
Conditions We Can Refer You For
Anxiety Disorders
Generalised anxiety disorder, panic disorder, social anxiety, health anxiety, and specific phobias — particularly where symptoms are severe, treatment-resistant, or significantly impacting daily functioning.
Depression
Moderate to severe depression, treatment-resistant depression, and cases with significant functional impairment. A psychiatrist can access specialist treatment options and therapies beyond standard GP care.
OCD
Obsessive compulsive disorder requiring specialist assessment, specialist clinical treatment, or structured ERP (Exposure and Response Prevention) therapy not available through standard NHS Talking Therapies pathways.
PTSD & Trauma
Post-traumatic stress disorder and complex PTSD, including trauma from childhood adverse experiences, accidents, abuse, or occupational trauma. Specialist psychiatrists can offer EMDR, trauma-focused CBT, and comprehensive treatment plans.
Bipolar Disorder
Suspected or confirmed bipolar disorder requiring specialist diagnosis and clinical management. Bipolar disorder must be managed by a consultant psychiatrist, as it requires specialist assessment and ongoing clinical monitoring.
Personality Disorders
Borderline personality disorder, emotionally unstable personality disorder, and other complex presentations where specialist assessment and structured therapy programmes are required beyond what primary care can provide.
Eating Disorders
Anorexia nervosa, bulimia nervosa, binge eating disorder, and ARFID requiring specialist psychiatric assessment, monitoring, and treatment planning. Eating disorders carry the highest mortality of any psychiatric condition.
Treatment Reviews
Second opinion on current psychiatric treatment, review of treatment-resistant cases, and specialist assessment where a fresh clinical perspective is needed on your care plan.
Burnout & Stress
Severe occupational burnout, work-related stress with psychiatric symptoms, and cases where the boundary between burnout and clinical depression or anxiety requires specialist assessment to determine the correct treatment pathway.

What Is a Psychiatry Referral Letter?
A psychiatry referral letter is an official document written by a UK-registered doctor that refers you to a consultant psychiatrist for specialist assessment. Psychiatrists are medical doctors — unlike psychologists or therapists, they can provide formal diagnoses and access the full range of clinical treatment options to manage complex or treatment-resistant mental health conditions. They are the appropriate specialist for conditions including bipolar disorder, severe depression, personality disorders, psychosis, and complex anxiety.
Many private psychiatrists accept self-referrals from self-pay patients. However, if you are using private medical insurance, a GP referral letter is a standard requirement. AXA Health requires a GP referral before authorising any consultant psychiatrist assessment. Bupa operates a Direct Access pathway for some mental health conditions, though this does not always apply to consultant psychiatrist appointments — check your specific policy before assuming direct access applies. Aviva follows the standard GP referral pathway for specialist mental health care.
Even where a referral is not strictly required, a well-written clinical letter meaningfully improves your first appointment. It gives the psychiatrist your full symptom history, prior treatments and their outcomes, and relevant medical background — information that would otherwise take much of a 50-minute consultation to establish. This means more time for assessment and treatment planning from the outset.
FastDoc referral letters are issued by GMC-registered doctors, written as open referrals to the specialty of psychiatry — the format preferred by AXA and most private hospitals — and delivered to your email as a PDF, same day.

Frequently Asked Questions
Do I need a referral letter to see a private psychiatrist?
It depends on how you are accessing private psychiatry. If you are self-paying and booking directly with an independent private psychiatrist, most will accept self-referrals without a GP letter. However, if you are using private health insurance, a GP referral letter is typically required. AXA Health requires a GP referral before authorising any consultant psychiatrist assessment. Aviva follows the standard referral pathway for specialist mental health care. Bupa operates a Direct Access pathway for some mental health presentations, though this does not always apply to consultant psychiatrist consultations — check your specific policy. A FastDoc referral letter from a GMC-registered doctor meets all standard insurer requirements.
What is the difference between a psychiatrist and a psychologist?
A psychiatrist is a medical doctor who has completed specialist training in psychiatry. The key distinction is that psychiatrists are medical doctors who can provide formal diagnoses and manage clinical treatment — they are the appropriate specialist for conditions requiring specialist management or formal diagnosis, including bipolar disorder, severe depression, psychosis, ADHD, and complex anxiety. A clinical psychologist holds a doctorate in clinical psychology but is not a medical doctor. Psychologists are highly trained in psychological assessment and therapy — CBT, EMDR, DBT — and are excellent for conditions that respond well to talking therapy. For most conditions, you may benefit from seeing both: a psychiatrist for diagnosis and treatment planning, and a psychologist for ongoing therapy.
Does AXA Health cover private psychiatry?
Yes, most AXA Health policies that include mental health cover will cover consultant psychiatrist assessments, subject to a GP referral letter and pre-authorisation. AXA prefers an open referral — addressed to the specialty of psychiatry rather than a named consultant — which is how FastDoc referral letters are written. Contact AXA with your referral letter before booking to receive your pre-authorisation code. Coverage limits, session numbers, and inpatient terms vary by policy — check your policy documents carefully.
Does Bupa cover private psychiatry?
Many Bupa policies include mental health cover. Bupa operates a Direct Access pathway for some mental health conditions, which can allow you to start care without a GP referral. However, for consultant psychiatrist appointments specifically, some Bupa policies still require a referral — particularly for complex conditions or inpatient care. Contact Bupa before booking to confirm whether a GP referral is needed for your specific policy and presentation.
How long is the NHS wait to see a psychiatrist?
NHS waiting times for specialist psychiatric care vary significantly by location and condition, but they are among the longest in the NHS. Community mental health team waits can stretch close to two years for those waiting the longest — people are 12 times more likely to wait over 18 months for mental health treatment than for equivalent physical health care. Over 1.7 million people are currently on NHS mental health waiting lists in England. With a FastDoc referral letter, a private consultant psychiatrist can typically see you within days to a few weeks.
How quickly will I receive my FastDoc referral letter?
Same day if you order before 8pm on the Standard service at £44.95. Our Premium service at £59.95 puts you at the top of the queue for fastest processing. Once a GMC-registered doctor has reviewed your case and issued the referral, it is sent directly to your email as a PDF — ready to present to your psychiatrist or insurer immediately.
What conditions can you refer me for?
FastDoc can issue psychiatry referral letters for a wide range of conditions including anxiety disorders, depression, OCD, PTSD and complex trauma, bipolar disorder, personality disorders, eating disorders, severe burnout, and treatment reviews for treatment-resistant cases. For ADHD assessment referrals specifically, please see our dedicated ADHD referral page which covers the ADHD-specific pathway in full detail.
Will a private psychiatrist's diagnosis be accepted by my GP?
In most cases, yes. A diagnosis made by a GMC-registered consultant psychiatrist following recognised assessment standards is clinically valid and should be accepted by your GP for ongoing treatment. However, for conditions requiring ongoing specialist clinical management — particularly complex conditions such as bipolar disorder — your GP may refer back to NHS secondary care services. It is always worth discussing ongoing care arrangements with your private psychiatrist at your initial consultation.
What if the doctor doesn't think I need a psychiatry referral?
If our GMC-registered doctor reviews your case and determines that a psychiatry referral is not clinically appropriate, you will receive a full refund and an explanation. We will not issue referrals that are not medically justified. In some cases, the doctor may suggest that a psychologist, therapist, or a different specialist would be more appropriate for your presenting symptoms.
Is my information kept confidential?
Yes. All information you provide is encrypted, securely stored, and only accessed by the doctor reviewing your case. FastDoc is fully GDPR compliant and never shares your information with third parties without your explicit consent. Mental health information is handled with particular sensitivity and the same strict confidentiality as all other medical data.
